    WHAT ARE WE OFFERING? 

    The Barossa Hills Fleurieu Local Health Network is seeking compassionate and motivated Community Support Workers to provide direct, personalised care to aged, frail, or disabled individuals who wish to remain living independently in their own homes. Working under a Consumer Directed Care model, you will support clients with personal care, meal preparation, cleaning, transport, and social engagement, enabling them to maintain control over their care and lifestyle choices. 

    This role requires flexibility, empathy, and the ability to work autonomously while collaborating with clients, their families, and other health professionals.     HOW WILL YOU CONTRIBUTE? 

    You will………. 

    • Provide personalised in-home and residential support to clients, including personal care, household tasks, meal preparation, social support, and assistance with mobility or therapy needs. 
    • Follow and contribute to individual care plans, monitoring client wellbeing and reporting changes in health or functional status to coordinators.  
    • Maintain a safe, clean, and organised environment, including equipment, aids, and client living spaces.  
    • Work collaboratively within a multi-disciplinary team, participating in training, quality improvement activities, and client service reviews.  
    • Comply with workplace health and safety standards, infection control, confidentiality, and continuous improvement initiatives.
