Disability support worker-Full time

We are seeking experienced Disability Support Workers to provide high-quality, person-centred care to clients in accordance with individual care plans.

The role involves delivering structured care services, including assistance with activities of daily living, personal care, mobility support, and social participation. Responsibilities also include supporting medication administration, assisting with domestic activities such as meal preparation, and providing specialised care including PEG feeding where required.

The position requires the ability to support clients with complex needs in a professional care environment, ensuring their safety, wellbeing and independence are maintained.

Your duties:

1. Deliver person-centered care aligned with individual care plans, including assistance with daily living activities, personal care, mobility support and social participation.
2. Implement personalized care plans to achieve individual goals
3. Encourage and assist clients with daily activities, home care, personal care, social inclusion and community access as required
4. Promote and assist clients to live an active, healthy and positive lifestyle
5. Provide client with emotional support
6. Transport clients as required
7. Maintain accurate records and documentation
8. Ensure NDIS compliance in all tasks
9. Follow all organizational policies and procedures

About you:

1. Certificate III in Individual Support (Ageing or Disability) or equivalent qualification or one year of relevant experience in aged care or disability care
2. Demonstrated experience delivering care in accordance with individual care plans
3. Experience supporting clients with complex care needs, including implementation of positive behaviour support plans
4. Strong communication and interpersonal skills, with the ability to work collaboratively within a care team
5. Experience in providing personal care, mobility support and assistance with activities of daily living
6. Experience in specialised care, including PEG feeding, is highly desirable
7. NDIS Worker Orientation Module (or willingness to complete)
8. Valid Australian Driver’s Licence
9. NDIS Worker Screening Check
10. National Police Check
11. Current First Aid and CPR certification
12. Manual Handling certification
13. Assist with Medication certification
