    The Family Support Workers' responsibilities will include and not limited to:

    • Assist with personal hygiene tasks such as washing hands, bathing/showering, brushing teeth, and getting dressed.

    • Adapt assistance based on the individual guest's needs and preferences.

    • Aid guests in using mobility equipment like wheelchairs, walking sticks, and walking frames.

    • Assist with mobility aids such as splints, braces, and shower chairs.

    • Support guests in navigating ramps, stairs, and transportation.

    • Accompany guests on shopping trips to supermarkets, cafes, and restaurants.

    • Perform shopping tasks on behalf of guests as needed.

    • Prepare and cook meals for guests, ensuring dietary preferences and restrictions are considered.

    • Clean and maintain rooms, including vacuuming, mopping, and making beds.

    • Report any maintenance issues to the house manager promptly.

    • Assist guests in participating in a variety of in-house and community-based activities.

    • Collaborate with guests to ensure their chosen activities align with their interests and preferences.

    • Take responsibility for the occupational health and safety of oneself, team members, contractors, and visitors.

    • Report any occupational health and safety incidents in a timely manner.

    • Comply with workplace health and safety policies and procedures.

    • Consistently attend scheduled weekly work sessions, ensure a positive participant experience, and comply with NDIS regulations.

    • Responding to participant issues/incident reporting to ensure they are supported in a timely and professional manner.

    • Liaising with participant allocations team, client service managers, participant plan nominees, and participant’s next-of-kin, to gather accurate details of the participant and their Care Plans.

    • Coordinating with the Accounts & Payroll team, for shift verification and adjustments.

    • monitoring and reporting on the progress of clients

    • assessing community need and resources for health, welfare, housing, employment, training and other facilities and services with Support coordinator.

    • Transport participants to appointments and community access, as per participant requirements.

    • Relaying crucial updates and alerts to the Allocations Team.

    • Supporting an inclusive participant-centric culture and proactive approach.

    • Contributing to the provision of relevant information and reporting.

    • Contributing to the process of documentation and continuous improvement activities.

    • Providing information to and participating in different care teams as appropriate.

    Qualifications, Skills, Experience, and Value-Add:

    1. Qualification required Certificate III in Individual Support and or higher

    2. Minimum of 2 years working within the Disability services sector, preferred within Family Services;

    3. NDIS Workers Screening Check (Required)

    4. Ability to work independently or within a team;

    5. Advanced computer and literacy skills;

    6. Knowledge of community agencies, with the ability to identify and coordinate a range of appropriate services relevant to the needs of clients;

    7. Current unrestricted Drivers Licence and First Aid Certificate;

    8. Approved AFP

    Benefits:

    Successful candidates can anticipate a competitive remuneration package of AUD$74,000- AUD$76,000 per annum, plus superannuation, for a full-time permanent position spanning 38 hours per week.

    • Interview clients and assess the nature and extent of difficulties with the support Coordinator

    • Understanding participant NDIS Plans, goals, and Care Plans to ensure shift tasks are met.

    • Personal care, manual handling, and medication administration.
