Salary: AUD 77,000 + superannuation (commensurate with skills and experience)
Position Summary:
We are seeking a compassionate and organised Family Support Worker to assist clients and their families in navigating daily challenges by coordinating personalised care and support services. This role involves the planning, implementation, and monitoring of domestic, healthcare, and community support programs to promote client independence and wellbeing.
Key Responsibilities:
·Assess clients’ individual needs through interviews and consultations,and develop tailored support programs basedon their circumstances.
·Review and interpret relevant My Aged Care and NDIS plans; prepare submissions for renewals or reassessments.
·Coordinate and supervise support workers delivering domestic and community support, ensuring quality service delivery.
·Monitor clients’ progress in alignment with their support plans and provide regular reports to stakeholders, including healthcare professionals and family members.
·Maintain accurate and up-to-date client records, case notes, and support documentation.
·Liaise with multidisciplinary professionals—such as nurses, dietitians, podiatrists, psychologists, occupational therapists, speech therapists, physiotherapists, welfare workers, and exercise physiologists—to ensure holistic care.
·Ensure clients’ satisfaction with the support workers and allied health professionals involved in their care.
·Gather client feedback to enhance service delivery and respond to evolving needs and preferences.
Ǫualifications and Skills:
·Diploma or higher qualification in Health, Community Services, Disability, or related field.
·Minimum one (1) year of full-time relevant experience.
·Strong verbal and written communication skills.
·Proven ability to work effectively within a multidisciplinary and culturally diverse
team.
·Understanding of NDIS and Aged Care frameworks, and client-centered care principles.
