Summary:

We are seeking a compassionate and dedicated Support Worker to provide assistance and support to individuals in need. The ideal candidate will have excellent communication skills, a positive attitude, and a willingness to work with individuals from diverse backgrounds. This position is relevant in all industries and requires a commitment to helping others.

Responsibilities:

• Provide emotional and physical support to individuals in need
• Assist with daily living activities, such as bathing, dressing, and grooming
• Help individuals with meal preparation and feeding
• Administer medication as directed by healthcare professionals
• Accompany individuals to appointments and activities
• Maintain accurate records of services provided
• Communicate effectively with individuals, families, and healthcare professionals
• Monitor and report any changes in an individual's condition
• Provide a safe and comfortable environment for individuals
• Group facilitating

Based on an integrated service delivery system Focus Connect's programs are specifically tailored through five broad service areas: children, youth, family, aged and disability.

All candidates must meet the following criteria:

• Current and valid Working With Children Check clearance (or willing to obtain)
• Current and valid full Driver’s Licence and access to a comprehensively insured vehicle
• Current and valid First Aid Certificate (or willing to obtain)
• Cleared National Criminal History check (or willing to obtain)
• OR current and cleared NDIS Worker Check
• Experience working within Aged Care or Disability Services
• Ability to work within a team and also autonomously
• Working rights in Australia
• Smartphone to access rostering application
• Compassionate and patient demeanor
